How will I contribute
The Clinic Manager is responsible for interacting directly with the Member and family, Authorized Representatives (ARs), caretakers, and another authorized support person (s), as appropriate. The Clinic Manager is responsible for engaging with a multi-disciplinary care team to identify gaps in the Members care and ensure appropriate input is obtained to effectively coordinate all primary, behavioral, developmental, oral health, LTSS, CS, and other services that address social determinants of health (SDOS), regardless of setting, at a minimum. Responsible for coordinating with those individuals and entities to ensure a seamless experience for the member and the non-duplication of services Engage eligible members, Oversee the provision of ECM services, including the completion of the assessment, development of a care plan, and implementation of the care plan. Offer services where the member lives seek care, or finds most easily accessible and within managed care plan (MCP) guidelines Connect member to other social services and supports they may need, including transportation Advocate on behalf of members with healthcare professionals Use motivational interviewing, trauma-informed care, and harm-reduction approaches Coordinate with hospital staff on discharge plan Accompany member to office visits, as needed and according to MCP guidelines Monitor treatment adherence (including medication)Provide health promotion and self- management training Attend and participate in cultural competency training in accordance with Kings View policy and procedures.
What I bring
MA, MS, or Ph.D. in Psychology or related field maintain licensure as an LMFT, LCSW LPCC, Class C
Maintain insurability with KV insurance, electronic health information systems, and outcomes measures, guide staff to their potential to keep at caseload appropriately sized to serve an adequate number of members, and progress note timeliness. Follow all KV policies and procedures, administrative directives, all relevant county, state, and federal regulations, and any performance obligations contained in KV program contracts. Routine monitoring of electronic health systems, staff compliance with documentation requirements, and that staff have adequate skills in using EHR. Ability to ensure a positive and effective working relationship with partner agencies and that Kings Views business interests are monitored and maintained. Two years post-licensure experience in a healthcare setting and preferred one year of supervising/directing the activities of other clinical staff in a healthcare setting. Assess ongoing program strengths and weaknesses then provide appropriate development and monitoring of program procedures: positive leadership and clinical expertise in diagnosing, planning, and treating MH disorders.
