Home Based Therapeutic Support Staff (HBTS) (Finance)
To provide support and services consistent with what is outlined in the HBTS Plan and Individual profile. Services and supports include but are not limited to areas outlined below:
To support children with special health care needs and their families in developing a positive self image.
To treat each individual and their family as persons who have unique needs and wants.
To support children with special health care needs and their families to function to their fullest potential.
To encourage children with special health care needs and their families to maintain positive relationships.
To work with the HBTS support team in developing consistent and integrated support techniques, methods and programs.
To gather and submit data related to achieving HBTS goals in a timely manner.
Home-Based Worker are responsible to maintain the following contemporaneous records:
Bi weekly time sheets which are signed by a parent and staff
Daily Documentation sheets which specifies the date, time, location, duration and levels of participation of parent and child for each therapeutic objective.
Individual scoring (data) sheets for each objective listed in the childs treatment plan. Data is compiled daily and documents the childs response to treatment, the level of assistance provided by staff or parents, and all other objective information identified by the therapeutic consultant as relevant to the evaluation of care for each objective.
Weekly case presentation sheets that prepare the staff for a supervisory meeting with clinical staff. Case presentations provide staff an opportunity to summarize data and other relevant information for each goal. These documents are also used to record feedback and recommendations provided by clinical staff.
Mandatory Qualifications and Requirements:
Is at least 19 years of age.
High School Diploma or GED required. Associates or higher degrees preferred.
Two years experience working with children with special health care needs or a combination of experience, education, and training working with children with special health care needs and their families strongly preferred.
Willingness and ability to complete and perform CPR and Nonviolent Physical Crisis Intervention (NCPI).
Willingness and ability to complete HBTS Training Curriculum.
Valid Driver's License and acceptable driving record required.
Access to and provision of an insured automobile on an ongoing basis.
