Job Title: Residential Care Provider (RCP)

Job Code: T6001/Support Services Worker

Shift: PRN/Hourly (up to 19 hours per week average)

Job Hours: Times May Vary

Base Location: Rockdale County

Division/Department/Program: Adult MH & Supportive Housing Semi-Independent

Under direct supervision, assists with daily living activities at the person's home, in a daytime non-residential facility or a residential facility. Provides training of basic personal and social skills, simple academics, or work skills to consumers/students in a state school, hospital, juvenile detention facility, residence, group home, workshop, or supported employment environment.

Duties & Responsibilities:
Provides training, community integration and support services to residents to support individuals in restoring and developing skills for living independently and independently participating in social, interpersonal and community activities.Conducts individual and group sessions to train on skills to prepare for independent living to include household cleanliness, budgeting, and maintaining physical and mental health as appropriate according to their individualized treatment plans.Monitors the self-administration of individuals medications using agency protocols.Participates as a member of individuals treatment team by observing individuals for any changes in behavior or health, communicating with community support teams, participating in treatment team meetings, and providing appropriate feedback, and assisting with transition and discharge processes.Assists with semi-independent enrollment processes and schedules progress meetingsProvides transportation to and from activities and appointments utilizing the Technicians personal vehicle or a 12-passenger van when necessaryAssists individuals in the pursuit of recreational, social and community activities based on their preferences and individualized treatment plans.Assists and encourages individuals in the establishment of social networks.Ensures each individuals rights are protected.Treats individuals with dignity and respectMaintains the residence in a clean and organized condition as individuals discharge and preparing units for new enrollments.Collaborate with Supervisor to resolve property damage or maintenance concerns.Other related duties as assigned.

Minimum Qualifications:
High school diploma/GED and six (6) months of relevant job-related training or experience.

Preferred Qualifications:
Must have at least one years experience working with the Mental Health population.Must be 21 years of age, have a valid Georgia Driver's License, and a clean motor vehicle report.

Requirements/Competencies:
Must pass a pre-employment drug screen and subject to random drug screens.Must have a valid Drivers License and a clean Motor Vehicle Record in accordance with company policy.Must pass a criminal background check (including fingerprinting).Requires TB Evaluation at pre-employment and annually.Must be able to lift 20 pounds.Requires long periods of sitting/standing.Must have the following technical skills Word, Excel and Electronic Health Record (Care Logic).Must be willing to transport residents in the company van.Must demonstrate the ability to maintain professional boundaries with clients in a residential treatment setting.Must be able to lift and carry groceries and supplies and move furniture for cleaning.Must be able to assist disabled individuals on/offvehicle and secure seatbelts.Must be able to use a gait belt and mechanical lift to lift and position individuals, as needed.
Note: Some positions may require a valid driver's license.
