We are seeking a dedicated and compassionate Family Medicine Nurse Practitioner to join our dynamic, multispecialty clinic. This role involves delivering comprehensive primary care to patients ages 13 and up, including Medicare, Medicaid, and commercially insured individuals. Providers manage a diverse range of cases, from routine physicals and well-woman exams to chronic disease management and preventive care.
The ideal candidate will be comfortable managing 16-20 patients per day and experienced in completing FMLA documentation and assessments. Daily responsibilities include working closely with a dedicated 1:1 support staff and a desk nurse to ensure efficient, patient-centered care. As part of a team of 43 staff and 23 providers across multiple specialties, this role offers built-in access to specialty referrals within the clinic.
Clinic hours are Monday-Friday, 8:00 AM to 5:00 PM, with a rotating Saturday morning schedule (8:00 AM to 12:00 PM). Experience in a high-volume outpatient setting, a commitment to team-based care, and a proactive approach to clinical documentation will distinguish top candidates.
Position Key Accountabilities:
1. Performs relevant patient assessments and physical examinations.
2. Coordinates, directs, and implements patient's plan of care.
3. Orders, conducts, and interprets pertinent laboratory and diagnostic studies and procedures.
4. Makes medical recommendations and/or referrals.
5. Formulates, implements, and evaluates an outcome-based treatment plan.
6. Maintains appropriate records detailing the patient's treatment plans and outcomes.
7. Facilitates patient education relating to medical processes, medication, nutrition, and health promotion.
8. Collaborates with patients, families, other clinicians, and referral services to assist in the patient's recovery.
9. Other duties as assigned.
Certification/Skills:
Minimum Education:
Masters in Nursing as NP or Masters degree in Nursing with completion of a post-masters Nurse Practitioner Certification Program.

Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
