Fall River Health Services is looking for a Provider Rounding Nurse for our Long-Term Care facility. In this role you will support daily or scheduled provider rounds by coordinating resident care, assisting with clinical assessments, and ensuring timely documentation and follow-through on medical recommendations. This role acts as a liaison between the interdisciplinary team, providers, residents, and families to enhance care quality, continuity, and communication. Schedule: 40 hours a week. Generally day shift must be flexible based on resident and provider availability. Working weekends and holidays on the floor maybe required. Responsibilities: Coordinate and participate in provider rounds (physician or NP/PA) for residents across the facility.Prepare for rounds by reviewing charts, labs, and recent clinical updates.Assist in assessments and documentation during provider visits, ensuring accurate and timely entries into the electronic health record (EHR).Communicate provider recommendations to the care team and ensure follow-through on orders, treatments, labs, and referrals.Facilitate communication between providers, nursing staff, and families to support collaborative care planning.Monitor residents for changes in condition and escalate concerns promptly to providers.Assist in discharge or care plan meetings as needed.Support medication management by ensuring MARs are accurate and up-to-date.Participate in quality improvement initiatives, regulatory readiness, and infection control practices.Advocate for resident dignity, comfort, and personalized care during all interactions.Update care plan as needed.Conduct assigned audits to ensure compliance with company policies and regulatory standards. The average pay is $27.00-$34.00/hour, depending on experience
