Primary Purpose: The Discharge Planning Assistant plans and facilitates safe discharges for patients utilizing clinical information and effective interpersonal skills.
Knowledge and Skills
Education: Minimum Required: High School Diploma or GED. Preferred: Bachelor's Degree in Social Work/Nursing
Years of Experience: Minimum Required: No experience required. Preferred: 1 - 2 Years equivalent experience
Required Skill and Knowledge: Knowledge of discharge planning and home health care preferred. Strong customer service skills and attitude. Ability to effectively communicate with and gain cooperation of social work team regarding discharge planning information. Effective written and verbal communication skills required for establishing and maintaining relationships and maintaining an accurate and complete medical record. Ability to interpret medical data as documented. Strong organizational skills to manage multiple, complex activities with tight deadlines, establish priorities and manage time effectively. Effective conflict resolution and negotiation skills. Knowledge and training in multiple computer systems required to perform position responsibilities (i.e. Allscripts/EC IN, Access Anywhere, Microsoft Office, etc.).
