Population Health Coordinator - 749 University Row (Healthcare)

We are seeking a Population Health Coordinator to:

  • Facilate program development and communication strategies und the direction of Population Health Leadership.
  • Develop and maintain program and department documentation and communication timelines, project plans, status reports and accomplishments.

Education:

Minimum – Bachelor's degree in Health Care Administration, Public Health, Social Work, Business Administration, Economics, Industrial Engineering or related field. Four (4) years of relevant process improvement and project coordination experience may be considered in lieu of degree in addition to the experience below.

Work Experience:

Minimum – Two (2) years of process improvement and project coordination experience

Preferred – Two (2) years of process improvement and project coordination experience in a health care setting

Required Skills, Knowledge, and Abilities:

  • Demonstrated experience managing multiple complex initiatives across organizational boundaries in close collaboration with leaders
  • Ability to translate strategies and goals into the design and implementation of systems and processes
  • Strong organizational, decision-making and problem-solving skills
  • Excellent facilitation, conflict resolution and interpersonal skills
  • Demonstrated success in leading or facilitating health care improvement
  • Ability to generate professional, clearly written communication that is tailored to the audience
  • Demonstrated ability to identify and effectively address dynamics that threaten success of a project or present barriers to implementation
  • Demonstrated success in leading through indirect authority

Physical Requirements:

Sedentary: Ability to lift up to 10 pounds maximum and occasionally lifting and/or carrying such articles as dockets, ledgers and small tools.  Although a sedentary job is defined as one, which involves sitting, a certain amount of walking and standing is often necessary in carrying out job duties.  Jobs are sedentary if walking and standing are required only occasionally and other sedentary criteria are met.

Work Schedule:

This is a full-time, 1.0 FTE position that is scheduled to work Monday through Friday, 8:00 am – 5:00 pm. Hours may vary based upon the operational needs of the department.
